Renewal Rehab is seeking a compassionate Physical Therapist Assistant to support high-quality rehab services for seniors in skilled nursing and post-acute care. In this role, you will implement treatment plans under the direction of a Physical Therapist, focusing on restoring function, mobility, and independence. Collaborate with an interdisciplinary team, document patient progress, and provide education to patients and families. Responsibilities Implement PT treatment plans under supervision of a licensed Physical Therapist Assist with therapeutic exercises, functional mobility, balance, and gait training Observe, measure, and document patient progress and responses to treatment Communicate effectively with PTs, nursing, and interdisciplinary team members Educate patients and families on home exercise programs and safety Maintain accurate, timely, and compliant clinical documentation Ensure safe use and maintenance of equipment and treatment areas Contribute to care planning and discharge recommendations Support a patient-centered, respectful environment for seniors Adhere to regulatory, ethical, and facility standards of care Required Skills Therapeutic exercise implementation Gait and balance training Functional mobility training Therapeutic modalities application (as allowed by state) Geriatric rehabilitation techniques Patient assessment and observation Clinical documentation in EMR systems Patient and family education Interdisciplinary team collaboration Knowledge of SNF/post-acute regulations and rehab guidelines
