Job Description
Renewal Rehab is seeking a compassionate Physical Therapist Assistant to support high-quality rehab services for seniors in skilled nursing and post-acute care settings. Working closely with Physical Therapists, you will implement individualized treatment plans, assist with therapeutic exercises and mobility training, document progress, and educate patients and families. You'll collaborate with an interdisciplinary team in a patient-centered, respectful environment that values integrity, mentorship, and open communication. Responsibilities Implement treatment plans under the direction and supervision of a Physical Therapist Provide therapeutic exercises, gait training, and functional mobility interventions for senior patients Assist with balance, strength, and endurance training to improve safety and independence Accurately document treatments, patient progress, and outcomes in the medical record Educate patients, families, and caregivers on exercises, safety, and use of assistive devices Collaborate with an interdisciplinary care team to support individualized care plans Monitor patient response to therapy and report changes to the supervising Physical Therapist Maintain compliance with state practice regulations, payer guidelines, and company policies Contribute to discharge planning and recommendations for continued services or equipment needs Participate in ongoing professional development, mentorship, and quality improvement initiatives Required Skills Therapeutic exercise implementation Gait and balance training Functional mobility and transfer training Use of assistive devices and durable medical equipment Geriatric rehabilitation techniques Therapy documentation and EMR use Patient and family education Pain management modalities within PTA scope Knowledge of skilled nursing/post-acute care regulations Safety, fall prevention, and infection control
