Job Description

Renewal Rehab is seeking a compassionate Physical Therapist Assistant to support seniors in skilled nursing and post-acute care settings. In this on-site role, you'll implement PT treatment plans to improve mobility, strength, balance, and functional independence, while documenting progress and collaborating closely with an interdisciplinary team. Responsibilities Deliver compassionate, evidence-based physical therapy interventions under PT supervision for seniors in skilled nursing and post-acute settings. Implement individualized treatment plans to improve mobility, strength, balance, and functional independence. Accurately document treatments, patient progress, and outcomes in the EMR in a timely and compliant manner. Educate patients, families, and caregivers on exercises, safety, and strategies to maintain functional gains. Collaborate with PTs, nursing, and interdisciplinary team members to support care plans and discharge goals. Adhere to regulatory, ethical, and company standards, promoting safety, integrity, and quality outcomes. Assist with caseload management to maintain efficient schedules and patient-centered care. Participate in ongoing training, mentorship, and professional development opportunities. Required Skills Therapeutic exercise implementation Gait and balance training Functional mobility training Therapeutic modalities (heat, cold, e-stim) Documentation in EMR systems Patient and family education Care planning in SNF/post-acute settings Vital signs monitoring and safety awareness Interdisciplinary team collaboration Knowledge of Medicare and rehab regulations
