Renewal Rehab is seeking a compassionate Physical Therapist Assistant to support high-quality rehabilitation services for seniors in skilled nursing and post-acute care settings. The PTA will implement treatment plans under the direction of a Physical Therapist, focusing on improving mobility, function, and quality of life. Responsibilities include assisting with evaluations, providing therapeutic exercises and modalities, documenting progress, and collaborating closely with an interdisciplinary care team. Responsibilities Implement physical therapy treatment plans under supervision of a Physical Therapist Provide therapeutic exercises, gait training, and balance activities for senior patients Assist with functional mobility, transfer training, and safety education Accurately document treatments, patient progress, and response to interventions Communicate with PTs, nursing, and interdisciplinary team regarding patient status Educate patients and families on home exercises and functional strategies Maintain compliance with federal, state, and facility regulations and documentation standards Participate in care conferences and discharge planning as needed Contribute to a patient-centered, respectful, and collaborative clinical environment Support quality improvement and clinical best practices within the facility Required Skills Therapeutic exercise implementation Gait and balance training Functional mobility and transfer training Therapeutic modalities (e.g., heat, cold, e-stim) Geriatric rehabilitation Skilled nursing/post-acute documentation Electronic medical record (EMR) documentation Patient and family education Knowledge of Medicare and reimbursement guidelines Interdisciplinary team collaboration
