Renewal Rehab seeks a compassionate Physical Therapist Assistant to support seniors in skilled nursing and post-acute care settings. Working under the direction of a Physical Therapist, you will provide evidence-based treatments to improve mobility, strength, and functional independence, while delivering patient-centered care. You'll collaborate with an interdisciplinary team, maintain thorough documentation, and contribute to quality outcomes. Responsibilities Provide direct patient care under supervision of a Physical Therapist for seniors in skilled nursing and post-acute settings Implement individualized treatment plans to improve mobility, strength, balance, and functional independence Accurately document treatments, patient progress, and outcomes in the EMR in a timely manner Collaborate with PTs, nurses, and interdisciplinary team members to support safe discharge and care transitions Educate patients and families on exercises, safety, and use of assistive devices Assist with screenings and re-assessments, reporting changes in patient status to the supervising PTMaintain compliance with state, federal, and company policies and quality standards Contribute to a positive, respectful, and patient-centered culture within the rehab team Required Skills Therapeutic exercise implementation Gait training and balance interventions Functional mobility training (transfers, bed mobility) Use of modalities (e.g., heat, cold, e-stim as allowed by state) Geriatric and fall-prevention practices Rehabilitation documentation and EMR proficiency Patient and family education techniques Knowledge of Medicare and reimbursement guidelines Interdisciplinary team collaboration Time management and caseload prioritization
