Renewal Rehab is seeking a compassionate Physical Therapist Assistant to provide evidence-based treatment for seniors in our skilled nursing and post-acute care settings. In this role, you'll deliver patient-centered interventions under the supervision of a Physical Therapist, focusing on mobility, strength, balance, and functional independence. You'll collaborate closely with an interdisciplinary team, maintain accurate documentation, and educate patients and families. Responsibilities Deliver individualized physical therapy interventions under PT supervision for seniors in skilled nursing and post-acute settings. Support evaluations and progress notes through accurate, timely documentation in the EMR system. Collaborate with an interdisciplinary care team to optimize functional outcomes and safe discharge planning. Educate patients and families on mobility, safety, and home exercise programs to promote long-term independence.
