Renewal Rehab is seeking a compassionate Physical Therapist Assistant to support high-quality rehab services for seniors in skilled nursing and post-acute care settings. Responsibilities Provide physical therapy interventions under the supervision of a licensed Physical Therapist Assist in implementing individualized treatment plans to improve mobility, strength, and functional independence Accurately document treatments, patient progress, and outcomes in the medical record Educate patients and caregivers on exercises, safety, and use of assistive devices Collaborate with PTs, OTs, nursing, and other interdisciplinary team members to coordinate care Monitor patient response to treatment and report changes in status to the supervising PTAdhere to all regulatory, ethical, and company standards of care and documentation Participate in care conferences and contribute to discharge planning Maintain a safe, clean treatment environment and proper use of equipment Engage in ongoing professional development and training offered by Renewal Rehab Required Skills Therapeutic exercise implementation Gait training and balance interventions Functional mobility training (transfers, bed mobility) Use of assistive devices and durable medical equipment Patient and caregiver education Electronic medical record (EMR) documentation Knowledge of geriatric and post-acute rehabilitation Therapeutic modalities (e.g., heat, ice, ultrasound as allowed by state law) Range of motion and strengthening techniques Infection control and safety procedures
