Working under the direction of a Physical Therapist, you will implement treatment plans, lead therapeutic exercises, document progress, and educate patients and families. Responsibilities Implement physical therapy treatment plans under PT supervision Provide therapeutic exercises, balance and gait training to seniors Monitor, document, and report patient progress and response to treatment Assist with patient assessments and adjust interventions per PT direction Educate patients, families, and caregivers on home exercise programs Ensure compliance with clinical, regulatory, and documentation standards Collaborate with an interdisciplinary team to support care plans Maintain safe treatment environments and proper use of equipment Contribute to discharge planning and functional goal attainment Participate in ongoing training and professional development activities Required Skills Therapeutic exercise implementation Gait and balance training Functional mobility training Geriatric rehabilitation techniques Skilled nursing/post-acute care documentation Use of therapy equipment and modalities per PTA scope Patient and family education Electronic medical record (EMR) documentation Safety and fall prevention in senior care Interdisciplinary care coordination
