Renewal Rehab is seeking a compassionate Physical Therapist Assistant to support delivery of high quality rehab services for seniors in skilled nursing and post acute care settings. Working under the direction of a Physical Therapist, you will carry out treatment plans, lead therapeutic exercises, document progress, and educate patients and families to maximize safety, mobility, and independence. Responsibilities Implement physical therapy treatment plans under PT supervision Provide therapeutic exercises, gait and balance training, and functional mobility activities Assist with transfer training and use of assistive devices to promote safety and independence Accurately document treatment, patient progress, and outcomes in the medical record Communicate effectively with PTs, nurses, physicians, and families regarding patient status Educate patients and caregivers on home exercise programs and safety strategies Participate in care planning, interdisciplinary team meetings, and discharge planning Ensure compliance with state, federal, and facility regulations and ethical standards Maintain treatment areas and equipment in a clean, safe condition Support a patient-centered, respectful environment for seniors in skilled nursing and post-acute care Required Skills Therapeutic exercise implementation Gait and balance training Functional mobility training Geriatric rehabilitation Skilled nursing/post-acute care experience Patient and family education Clinical documentation and EMR use Transfer training and safe patient handling Use and fitting of assistive devices Interdisciplinary team collaboration
