Renewal Rehab is seeking a compassionate Physical Therapist Assistant to support seniors in skilled nursing and post-acute care settings. Working under the direction of a Physical Therapist, you will deliver individualized treatment plans focused on mobility, strength, balance, and functional independence. Responsibilities Deliver physical therapy interventions under the direction and supervision of a licensed Physical Therapist Implement individualized treatment plans to improve strength, mobility, balance, and functional independence for seniors Accurately document treatments, patient progress, and responses in the electronic medical record Educate patients, families, and caregivers on exercises, safety, and functional strategies for home and facility settings Collaborate with PTs, nursing, and interdisciplinary team members to support care plans and achieve quality outcomes Adhere to all regulatory, ethical, and company standards while maintaining a safe, patient-centered environment Participate in case conferences, care plan meetings, and quality improvement initiatives as needed Required Skills Therapeutic exercise implementation Gait training and balance interventions Functional mobility and transfer training Use of modalities (heat, cold, e-stim, ultrasound per state law) Documentation and EMR proficiency Patient assessment and progression within PT plan of care Knowledge of geriatric conditions and fall prevention Care planning in skilled nursing/post-acute settings Infection control and safety protocols Interdisciplinary team collaboration and communication
