Job Description
Renewal Rehab is seeking a compassionate Physical Therapist Assistant to support seniors in skilled nursing and post-acute care settings. As part of our collaborative Healthcare & Medical Services team, you will provide hands-on treatments under the direction of a Physical Therapist, helping patients improve mobility, strength, balance, and independence. Responsibilities Deliver skilled PTA interventions under supervision of a Physical Therapist for senior patients in skilled nursing/post-acute settings Follow and progress individualized treatment plans to improve mobility, strength, balance, and functional independence Accurately document treatments, patient responses, and progress in the electronic medical record in a timely manner Educate patients, families, and caregivers on exercises, safety, and strategies to maximize functional outcomes Collaborate with PTs, nurses, and the interdisciplinary team to support coordinated, patient-centered care Assist with functional assessments, discharge planning, and equipment recommendations as directed by the PTMaintain compliance with state practice acts, Medicare and facility regulations, and Renewal Rehab clinical standards Contribute to a positive, respectful culture focused on integrity, quality outcomes, and compassionate senior care Required Skills Therapeutic exercise implementation Gait and balance training Functional mobility training Treatment documentation (EMR) Patient and family education Range-of-motion and strengthening techniques Safety assessment and fall prevention Care planning and interdisciplinary collaboration Vital signs monitoring Knowledge of Medicare and rehab regulations
