Job Description

Renewal Rehab is seeking a compassionate Physical Therapist Assistant to provide high-quality, evidence-based care for seniors in skilled nursing and post-acute care settings. In this onsite role, you will implement individualized treatment plans under the direction of a Physical Therapist, focusing on mobility, strength, balance, and safety to enhance functional outcomes and quality of life. Responsibilities Deliver delegated physical therapy treatments to senior patients under PT supervision Implement individualized treatment plans to improve mobility, strength, and balance Document treatments, progress notes, and outcomes in EMR accurately and timely Educate patients, families, and caregivers on exercises, safety, and functional strategies Collaborate with PTs, nurses, and interdisciplinary team on care planning Monitor patient response, report changes in condition, and adjust interventions per PT direction Support safe discharge planning and equipment recommendations Uphold company values of integrity, respect, and patient-centered care Required Skills Therapeutic exercise for geriatric patients Gait training and balance interventions Functional mobility and transfer training Use of assistive devices and DMEDocumentation in EMR systems Patient and family education Care planning with interdisciplinary team Knowledge of SNF/post-acute rehab regulations Outcome measurement and functional assessments Infection control and safety procedures
