Job Description
Renewal Rehab is seeking a compassionate Physical Therapist Assistant to support high quality rehab services for seniors in skilled nursing and post acute care settings. Working under the direction of a Physical Therapist, you will implement individualized treatment plans, assist with therapeutic exercises and functional training, document progress, and collaborate with an interdisciplinary team. Our patient centered culture offers strong clinical resources, mentorship, manageable caseloads, and opportunities for growth while helping seniors improve mobility, independence, and quality of life. Responsibilities Implement PT treatment plans under supervision of a licensed Physical Therapist Provide therapeutic exercises, gait training, and functional mobility interventions Assist patients with safe use of assistive devices and adaptive equipment Observe, measure, and document patient progress and response to treatment Communicate patient status and recommendations to the PT and care team Educate patients, families, and caregivers on exercises and safety strategies Ensure treatments follow regulatory, ethical, and company standards Contribute to care planning in an interdisciplinary senior care environment Maintain accurate, timely clinical documentation in EMR systems Support a safe, respectful, and patient centered therapy environment Required Skills Therapeutic exercise techniques Gait training and balance intervention Functional mobility and transfer training Use and fitting of assistive devices Geriatric rehabilitation principles Skilled nursing/post acute care practice Patient and family education Clinical documentation and EMR use Vital signs monitoring and basic assessment Infection control and safety procedures
