Key Responsibilities Treatment Delivery:

Execute personalized physical therapy plans created by the supervising Physical Therapist.

Patient Interventions:

Guide residents through gait, transfer, balance, and therapeutic exercise training to improve functional mobility.

Interdisciplinary Collaboration:

Participate actively in care team meetings to coordinate holistic patient treatment.

Patient & Family Education:

Instruct residents and their families on safe mobility techniques and home exercise programs.

Documentation:

Maintain accurate, timely, and compliant daily treatment logs using electronic documentation systems.

PT Assistance:

Provide hands-on assistance to supervising PTs during complex tests and specialized patient evaluations. Qualifications Active, unrestricted Physical Therapist Assistant (PTA) license in the state of Illinois (or immediate license eligibility). Strong interpersonal skills and a passion for working with senior care and rehabilitation populations. New graduates are encouraged to apply!
