Renewal Rehab is seeking a compassionate Physical Therapist Assistant to support physical therapy services for seniors in skilled nursing and post-acute care settings. Working closely with supervising Physical Therapists, you will implement treatment plans, assist with therapeutic exercises, functional mobility training, and balance activities to promote safety and independence. You'll document patient progress, collaborate with an interdisciplinary care team, educate patients and families, and contribute to a positive, patient-centered environment focused on integrity, respect, and quality clinical outcomes. Responsibilities Implement treatment plans under the direction of a licensed Physical Therapist Provide therapeutic exercises, gait training, and balance activities to seniors Assist with functional mobility, transfers, and fall-prevention strategies Monitor and document patient progress and response to treatment in EMR systems Communicate effectively with PTs, nursing, and interdisciplinary care teams Educate patients and families on home exercise programs and safety Ensure treatments follow payer, regulatory, and facility guidelines Contribute to a collaborative, patient-centered senior care environment Participate in case conferences and care planning meetings Support quality improvement and best-practice clinical initiatives Required Skills Therapeutic exercise implementation Gait and balance training Functional mobility and transfer techniques Geriatric rehabilitation principles Skilled nursing/post-acute care documentation Use of electronic medical records (EMR) Patient and family education Body mechanics and safe patient handling Knowledge of Medicare and payer guidelines for rehab Interdisciplinary team collaboration
