Renewal Rehab is seeking a compassionate Physical Therapist Assistant to support seniors in skilled nursing and post-acute care settings. In this role, you'll deliver hands-on treatments under the direction of a Physical Therapist, focusing on mobility, strength, balance, and functional independence. You'll document outcomes, educate patients and families, and collaborate with an interdisciplinary team. Responsibilities Deliver physical therapy treatments under the direction of a licensed Physical Therapist for seniors in skilled nursing and post-acute settings. Assist with evaluations by gathering patient history, functional status, and mobility baselines. Implement individualized treatment plans focused on strength, balance, mobility, and fall prevention. Accurately document treatments, progress notes, and functional outcomes in the EMR. Educate patients, families, and caregivers on exercises, safety, and use of assistive devices. Collaborate with an interdisciplinary care team to support patient-centered goals and discharge planning. Monitor patient response, report changes in status to the PT, and adjust interventions within scope. Contribute to a culture of integrity, respect, and quality outcomes through evidence-based practice.
