Job Description

Renewal Rehab is seeking a compassionate Physical Therapist Assistant to support delivery of high-quality rehab services for seniors in skilled nursing and post-acute care settings. Working under the direction of a Physical Therapist, you'll implement treatment plans, assist with therapeutic exercises, document progress, and help patients improve mobility, safety, and independence. Our collaborative, patient-centered culture offers strong clinical resources, mentorship, manageable caseloads, and opportunities for professional growth while making a meaningful daily impact on seniors' quality of life. Responsibilities Implement PT treatment plans under supervision of a licensed Physical Therapist Assist patients with therapeutic exercises and functional mobility training Monitor, document, and report patient progress and response to treatment Educate patients and families on exercises, safety, and use of assistive devices Collaborate with PTs, nursing, and interdisciplinary team to coordinate care Ensure accurate, timely, and compliant clinical documentation Maintain safe treatment environment and adhere to infection control standards Support discharge planning and recommendations for continued rehabilitation Contribute to outcome tracking and quality improvement initiatives Participate in professional development, in-services, and mentorship activities Required Skills Therapeutic exercise implementation Gait and balance training Functional mobility and transfer training Geriatric rehabilitation Skilled nursing/post-acute rehab documentation Use of assistive devices and DMEPatient and family education Care planning within interdisciplinary teams Outcome measurement and functional assessments Infection control and safety procedures
