Job Description

Renewal Rehab is seeking a compassionate Physical Therapist Assistant to provide high-quality, evidence-based therapy for seniors in skilled nursing and post-acute care settings. In this onsite role, you will implement individualized treatment plans under the direction of a Physical Therapist, focusing on mobility, strength, balance, and safety to improve functional outcomes and quality of life. You'll enjoy a collaborative, patient-centered culture, strong clinical support, manageable caseloads, and opportunities for ongoing professional development in a growing senior care therapy organization. Responsibilities Deliver skilled physical therapy interventions to senior patients under the direction of a Physical Therapist Support functional mobility, strength, balance, and safety for patients in skilled nursing and post-acute care Accurately document treatments, progress, and outcomes in accordance with facility and regulatory standards Educate patients, families, and caregivers on home exercises, safety, and functional strategies Collaborate with PTs, nurses, and the interdisciplinary team to support patient-centered care plans Contribute to a positive, respectful, and integrity-driven culture focused on quality outcomes Required Skills Therapeutic exercise implementation Gait training and balance interventions Transfer and mobility training Patient and family education Documentation in EMR systems Care plan implementation under PT direction Range of motion and strengthening techniques Functional mobility assessment Safety and fall-prevention techniques Interdisciplinary team collaboration
