Renewal Rehab is seeking a compassionate Physical Therapist Assistant to provide high-quality therapy services for seniors in skilled nursing and post-acute care settings. Working under the direction of a Physical Therapist, you will deliver individualized treatment plans focused on mobility, strength, balance, and fall prevention to enhance patients' functional outcomes and quality of life. Responsibilities Deliver physical therapy interventions under the direction of a licensed Physical Therapist for senior patients in skilled nursing and post-acute settings. Implement individualized treatment plans focusing on mobility, strength, balance, and fall prevention to improve functional outcomes. Accurately document treatments, progress notes, and outcomes in the electronic medical record in a timely manner. Communicate effectively with PTs, nursing, physicians, and families to support coordinated, patient-centered care. Educate patients and caregivers on home exercise programs, safety techniques, and use of assistive devices. Monitor patient response to treatment, report changes in condition, and contribute to care plan updates. Support a culture of integrity, respect, and clinical excellence while maintaining regulatory and company compliance. Participate in mentorship, in-services, and professional development opportunities to enhance clinical skills.
