We are a Physical Therapist owned and locally operated healthcare system offering skilled services through our Home Health, Hospice, Outpatient, and Home Caregiving divisions. This position offers unique flexibility and an opportunity to work with a variety of clientele needs in a laid-back, family oriented, client-centered environment day-to-day. As our programming and team continue to grow, we desire to have a network of per diem clinicians with flexibility to assist the families we serve in the area. Responsibilities:
Evaluation and assessment of the client's level of function by applying diagnostic and prognostic procedures. Formulating a plan-of-care which includes client-centered goal creation. Providing education to both the client, family, and caregiver(s) as available. Assists with the development and implementation of an interdisciplinary approach to treatment. Guides each client and instructs them in the prescribed therapeutic interventions. Instructs clients and (or) their family in the carryover of a home program. Completes all assigned documentation in a timely manner. Communicates openly with other Carefirst Rehab team members and physician(s) regarding the case as appropriate. Participates in Carefirst Rehab activities, in-services, and meetings as assigned. Completes discharge planning and all documentation relevant to the event.Clinical Expectations:
