Job Summary:
Fortis Health is seeking a skilled and compassionate Licensed Physical Therapist Assistant (PTA) to provide in-home therapy services under the supervision of a Registered Physical Therapist (PT). This role delivers direct patient care based on an established plan of care, monitors patient progress, and supports patients and caregivers in achieving safe, functional independence at home.
Duties/Responsibilities:
Provide physical therapy interventions in accordance with the physician-approved plan of care developed and supervised by the Registered Physical Therapist. Deliver treatments to relieve pain, restore function, and maintain optimal performance using approved therapy techniques, modalities, and procedures. Monitor patient progress toward goals and ensure compliance with physician orders. Observe, document, and report patient responses to treatment and any changes in condition to the PT and/or Director of Rehabilitation (DOR) or Director of Nursing (DON). Collaborate with the PT in reassessing patient functional status and contributing to plan of care updates. Educate patients and caregivers on exercises, safety precautions, mobility techniques, and discharge planning. Participate in care conferences and interdisciplinary meetings to ensure coordinated, high-quality care. Maintain accurate, timely documentation in accordance with agency policies and regulatory requirements. Participate in quality improvement initiatives and in-service education programs. Remain current with physical therapy trends, standards, and best practices. Physical Requirements Ability to sit, stand, bend, lift, and assist patients with transfers and mobility, with or without reasonable accommodation. Ability to travel to patient homes and drive for extended periods. Ability to move therapy equipment and supplies as needed. Regular use of a tablet or electronic device for documentation and communication.
