What You'll Do:
Provide personalized physical therapy under the direction of a Physical Therapist to help patients improve mobility, strength, balance and functional independence Implement patient-centered treatment plans designed to support recovery, safety, pain management, and long-term rehabilitation goals Deliver hands-on therapy interventions that enhance movement, endurance and overall quality of life for adult and geriatric patients Partner with interdisciplinary teams , caregivers, and nursing staff to support strong patient outcomes and continuity of care Quickly integrate with interdisciplinary teams, caregivers, and nursing staff to drive meaningful patient outcomes throughout your assignment Track patient progress, maintain accurate clinical documentation , and adapt treatment plans based on patient needs and rehabilitation goals

Qualifications:
Licensed Physical Therapist Assistant (PTA) or license eligible in the state of practice Graduate of an accredited Physical Therapist Assistant Program Interest or experience in skilled nursing, long-term care or geriatric rehabilitation Passion for patient-centered care and improving quality of life for older adults Willingness to travel to multiple TMC locations as needed
