This role involves providing personalized physical therapy services to a diverse population of seniors, ensuring they benefit from the highest standards of healthcare in their home settings. Responsibilities Provide physical therapy services under the supervision of a licensed Physical Therapist. Implement treatment plans devised by the physical therapist to improve patients' mobility and alleviate pain. Assist in patient assessments and regularly monitor their progress. Educate patients and their families on in-home care strategies and preventive measures. Ensure documentation and care delivery meet compliance standards. Collaborate with healthcare teams to optimize care plans.
