Traveler Requirements:

Must have at least 3 months of outpatient experience or 1 year of PT experience in other settings. HealthStream Modules will be required as a part of pre-placement requirements. Agencies will be reimbursed a 1-time payment with proof of completion certificates.

Responsibilities:

Provide outpatient physical therapy services in a patient-centered care environment. Support leadership in implementing supportive care models. Ensure documentation and treatment plans are accurate and timely in Raintree EMR. Collaborate with team members to achieve productivity targets of 55-63 patients per week.
