We are seeking a licensed Physical Therapist Assistant to deliver patient-centered care in a home health setting.

Key responsibilities include:

Following state laws and supervising physical therapists' treatment plans Instructing and assisting patients with exercises, gait, transfers, and safety training Monitoring and documenting patient responses and communicating with healthcare team members Educating patients, families, and caregivers on therapy techniques within scope

Qualifications:

Current license or certification in physical therapy assistance CPR certification Valid driver's license, access to reliable transportation, and ability to work in diverse home environments Willingness to work weekend rotations Preferred skills: Physical ability to perform patient mobility tasks such as lifting and transferring.
