Role:
Trust Home Care Inc is looking for a dedicated and compassionate Home Health Physical Therapy Assistant to join our growing team in Harford County. In this rewarding role, you'll make a meaningful difference by delivering hands-on rehabilitative care directly to patients in the comfort of their own homes. Responsibilities:
Implement physical therapy treatment plans under the supervision of a licensed Physical Therapist Provide in-home therapeutic exercises, gait training, and functional mobility assistance Monitor and document patient progress, reporting changes in condition to the supervising PT Educate patients and family members on home exercise programs and safety techniques Assist patients with activities of daily living (ADLs) to promote independence and recovery Maintain accurate and timely clinical documentation in compliance with home health regulations Coordinate care with interdisciplinary home health team members to ensure continuity of care
Requirements:
Active Maryland PTA license in good standing Graduate of an accredited Physical Therapist Assistant program Previous home health or outpatient PT experience preferred Valid driver's license and reliable transportation for home visits in the Bel Air, MD area Strong knowledge of rehabilitative techniques, therapeutic exercise, and patient safety Excellent communication, time management, and organizational skills Compassionate, patient-centered approach with a commitment to quality care
