Responsibilities include:
Evaluate and treat patients in their home environment Develop individualized, evidence-based treatment plans Improve performance with activities of daily living (ADLs) and instrumental activities of daily living (IADLs) Assess home safety and recommend adaptive equipment Educate patients and caregivers on treatment plans and fall prevention Document care using EMR Collaborate with physicians, nurses, therapists, and case managers to optimize patient outcomes Qualifications Required Graduate of a CAPTE-accredited Physical Therapy Assistant program Current Maryland Physical Therapist Assistant license (or eligibility) American Heart Association (AHA) or American Red Cross Basic Life Support (BLS) certification Strong communication, organization, and time management skills Preferred One year of home health experience Inpatient rehabilitation or skilled nursing experience Experience with EMR documentation Ability to work independently while managing a mobile caseload Benefits We invest in our clinicians' success with benefits that support your career, health, and well-being.
