LTAC/LTC

setting, supporting mobility and functional goals. Implement and monitor components of the plan of care, adjusting as directed by the supervising PT. Facilitate therapeutic interventions including therapeutic exercise, range of motion, strengthening, gait and balance training, and transfer techniques. Assist with patient positioning, mobility, and safety during therapy sessions; ensure proper use of assistive devices. Document patient progress, modify treatment notes as needed, and communicate concerns or changes to the supervising PT and interdisciplinary team. Educate patients and families on home exercise programs, safety plans, and carryover strategies to support recovery and functional independence. Requirements Ability to work effectively in a
