Job Description

Renewal Rehab is seeking a compassionate Physical Therapist Assistant to support seniors in skilled nursing and post-acute care settings. Working under the direction of a Physical Therapist, you will implement individualized treatment plans, guide therapeutic exercises and functional mobility training, and document patient progress. You'll collaborate closely with an interdisciplinary care team, educate patients and families, and help optimize safety, independence, and quality of life. Responsibilities Implement treatment plans under supervision of a licensed Physical Therapist Provide therapeutic exercises, gait training, and functional mobility interventions for seniors Monitor, document, and report patient progress and response to treatment Assist with patient assessments and contribute to care planning Educate patients, families, and caregivers on exercises, safety, and equipment use Collaborate with nursing, rehab, and medical staff to coordinate patient care Ensure accurate, timely, and compliant documentation in EMR systems Maintain a safe, clean treatment environment and proper equipment use Support discharge planning with home exercise and safety recommendations Participate in clinical meetings, in-services, and continuing education Required Skills Therapeutic exercise implementation Gait and balance training Functional mobility training Geriatric rehabilitation Skilled nursing/post-acute care treatment Patient and family education Rehabilitation documentation and EMR use Safe transfer and wheelchair management Range of motion and strengthening techniques Fall prevention and safety training
