Job Description

Renewal Rehab is seeking a compassionate Physical Therapist Assistant to join our Healthcare & Medical Services team supporting seniors in skilled nursing and post-acute settings. Under PT supervision, you'll deliver individualized treatments to improve mobility, strength, and function, while documenting outcomes and collaborating with an interdisciplinary team. Responsibilities Provide treatment to senior patients under the supervision of a Physical Therapist Implement individualized care plans to improve mobility, strength, and function Document treatments, progress notes, and outcomes accurately in EMR systems Collaborate with PTs, nursing, and interdisciplinary teams on patient care Educate patients and families on exercises, safety, and functional goals Support a patient-centered, compassionate approach consistent with Renewal Rehab's values Required Skills Therapeutic exercise implementation Gait and balance training Modalities application (e.g., heat, cold, e-stim) Documentation in EMR systems Functional mobility and transfer training Patient and family education Care planning with supervising PTKnowledge of SNF/post-acute regulations Infection control and safety practices Outcome measurement and reporting
