ABOUT CARE RESOURCES
Care Resources operates the PACE program (Program of All-inclusive Care for the Elderly) which is located in at 4150 Kalamazoo Avenue SE in Grand Rapids. Care Resources provides and coordinates all medical care and social services to eligible participants. We provide all physician and nursing care, prescription drugs, durable medical equipment, dental, eye and foot care, hospital care, medical transportation, home care, emergency, rehab, social and recreational activities at the Day Center. To be eligible for the PACE program, seniors must be 55 and older, a Kent County resident, able to live safely at home and meet the nursing facility level of care.
