Essential Functions:
Day to Day operations : As a St. Ann's Home Physical Therapy Assistant, you will provide direct patient care; evaluate case history, administer physical therapy treatments including physical agents to patients, working under the direction of and assistant to a Physical Therapist.
Duties/Responsibilities:
Assist Physical Therapist with initial evaluation on new patients to assess their current symptoms, establish goals. Observes patients during treatments and compiles and evaluates data on patient's responses to treatments and progress. Appropriately documents such in the medical record. Administers active and passive therapeutic exercises Continuously evaluates patients' progress and prepares reports that detail progress. Participates in Care Conferences, Team Meetings and communicates with members of the interdisciplinary team concerning patient treatment as necessary. Protects St. Ann's Residents privacy by keeping information secure and confidential.
Communication:
Keep regular, open communication with Physical Therapist and onsite clinical team on any needs of current or predicted changes of condition in regards to St.

Education and Experience Requirements:
Associates Degree and current Michigan license as a Physical Therapist Assistant; BLS certification. 2-5 years of related experience, or equivalent combination of education and experience Experience working in a long term care setting preferred Ability to work in a fast-paced environment with multiple competing demands for time and attention. Ability to be self-motivated to achieve desired organizational and Resident goals. Must not pose a direct threat to the health or safety of other individuals in the workplace. Excellent verbal communication skills with the ability to interact with a diverse group of associates, residents, and family members. Ability to apply reason, logic, and advanced problem-solving skills to resolve complex and/or sensitive issues. Excellent written communication skills including proper grammar and professional writing. Working Conditions:
Using hands and arms in handling, installing, positioning, and moving materials, and manipulating things. Is subject to call back during emergency conditions (e.g., severe weather, evacuation, post-disaster, etc.). Performing physical activities that require considerable use of your arms and legs and moving your whole body, such as climbing, lifting, balancing, walking, stooping, and handling of materials. Frequent exposure to extreme temperatures (Hot and Cold weather). Is involved in community/civic health matters/projects as appropriate. Attends and participates in continuing educational programs. Is subject to injury from falls, burns from equipment, odors, assaults from hostile residents, etc., throughout the work day, as well as reactions from dust, disinfectants, tobacco smoke, and other air contaminants. Physical Environment:
Must be able to move constantly throughout the workday. Must be able to speak and write the English language in an understandable manner. Must be able to cope with the mental and emotional stress of the position. Must function independently, and have flexibility, personal integrity, and the ability to work effectively with residents, personnel, and support agencies. Must meet the general health requirements set forth by the policies of this facility, which include a medical and physical examination. Must be able to push, pull, move, and/or lift a minimum of 25 pounds to a minimum height of 5 feet and be able to push, pull, move, and/or carry such weight a minimum distance of 50 feet. May be necessary to assist in the evacuation of residents during emergency situations.
