Job Description
Physical Therapist Assistant (PTA) | Home Visits | Holland Area Atrio Home Care, a partnership between Holland Home, Brio, and Resthaven, is seeking a compassionate and driven Physical Therapist Assistant (PTA) to join our Holland/Lakeshore team. In this role, you'll provide high-quality, one-on-one care to clients in their homes within approximately a 40-mile radius of Holland, MI. What You'll Do Travel to client homes or care facilities to complete follow-up therapy visits Deliver physical therapy services under the supervision of a licensed Physical Therapist, including implementing restorative care and home exercise programs Complete accurate clinical documentation and progress notes in MatrixCare, ensuring compliance with agency policies and HIPAA standards Collaborate with therapists and the interdisciplinary team to support excellent patient outcomes and smooth discharge planning Educate clients, families, and caregivers on therapy techniques and ongoing care needs Assist with therapy equipment management, program development, and participate in team trainings and goal setting initiatives We now have a lot of Ortho cases- selling point They only have routine follow-up visits, no oasis; helps with less medically complex pts 6 visits minimum or 30 visits per week
