Job Description
Renewal Rehab is seeking a compassionate Physical Therapist Assistant to support personalized, outcome-focused rehab programs for seniors in skilled nursing and post-acute settings. In this role, you will implement treatment plans designed by the Physical Therapist, focusing on mobility, strength, balance, and functional independence. Responsibilities include providing one-on-one interventions, documenting progress in EMR, educating patients and families, and collaborating with an interdisciplinary team. Responsibilities Implement PT treatment plans to improve strength, balance, mobility, and functional independence for senior patients. Provide one-on-one therapeutic exercises, gait training, and functional activities in skilled nursing and post-acute settings. Accurately document treatments, patient response, and progress in the electronic medical record (EMR). Educate patients and families on exercises, safety, and strategies to support continued progress and safe discharge. Collaborate with Physical Therapists, nursing, and interdisciplinary team members to support plan of care and patient goals. Monitor patient safety, apply appropriate assistive devices, and follow fall-prevention and infection-control protocols. Assist with outcome tracking and contribute to quality improvement initiatives within the rehab program.
