Lansing, MI Job Description General Purpose of Job :
Provides direct physical therapy services under the direction and supervision of a licensed Physical Therapist according to the physician approved written plan of care.Essential Duties :
This job description is intended to cover the minimum essential duties assigned on a regular basis. Caregivers may be asked to perform additional duties as assigned by their leader. Leadership has the right to alter or modify the duties of the position. Provides direct physical therapy treatment to patients according to instruction of Physical Therapist, and in accordance with the written plan of care Observes patient and reports to Physical Therapist and department personnel the patient's reaction to treatment and any changes in patient's condition Instructs patients, family and/or caregiver in proper techniques to assist patient in achieving maximum level of independence Maintains necessary records, progress notes, and conference notes, which are incorporated into the patient's clinical record within established time frame Maintains professional standards, compliance with State, Federal (CMS conditions of participation) and Joint Commission regulations, and infection control protocol by participating in in-services, staff meetings, process improvement teams, continuing education seminars when available, and adhering to the SHC expectations - standards of care guidelines Job Requirements General Requirements- PTA license to practice in State of Michigan
- Valid driver's license and automobile insurance
- Active American Heart Association BLS Certification upon hire Work Experience
- Two years acute care or home care experience Education
- Graduate of a Physical Therapy Assistant program
Specialized Knowledge and Skills Required:
- Demonstrated ability to work with patient, caregivers, and others in a variety of home care situation
- Demonstrated interpersonal skills to effectively and appropriately communicate with patients, families, physicians, and interdisciplinary team
- Demonstrates ability to use a keyboard as may be required to perform the essential duties of the job
- Demonstrated ability to assemble and document data in proper format
- Ability to enter/exit patient homes that may/may not be handicap accessible
