Renewal Rehab is seeking a compassionate Physical Therapist Assistant to provide high-quality rehabilitative care for seniors in skilled nursing and post-acute settings. Working under the direction of a Physical Therapist, you'll assist with individualized treatment plans, lead therapeutic exercises, document patient progress, and support safe mobility and functional independence. Responsibilities Assist Physical Therapist in implementing individualized treatment plans for senior patients Lead therapeutic exercises and functional mobility training under PT supervision Assist with gait training, balance activities, and transfer training for safety Monitor and document patient progress, response to treatment, and functional outcomes Educate patients and caregivers on exercises, safety, and use of assistive devices Collaborate with the interdisciplinary team to support discharge planning and goals of care Maintain accurate, timely clinical documentation per company and regulatory standards Ensure safe use of equipment and adherence to infection control and safety protocols Contribute to a positive, patient-centered environment focused on quality outcomes Participate in ongoing professional development and in-service education Required Skills Therapeutic exercise implementation Gait and balance training Functional mobility and transfer training Geriatric rehabilitation techniques Skilled nursing/post-acute care practices Patient and caregiver education Clinical documentation (EMR) Safety and infection control practices Use of assistive devices and DMEInterdisciplinary team collaboration
