Job Description

Physical Therapist (PT) or Physical Therapist Assistant (PTA): Full-Time Medical/Dental Benefits (BCBS) Paid Time Off Flexible Schedule Company Matched IRA Flex Spending Account Seeking a PT or PTA to fulfill a full-time position in our home healthcare and house calls divisions. Great Lakes Home Healthcare, House Calls, and Physical Therapy provides the complete continuum of rehabilitative care. Locally owned and operated in Traverse City since 2003, we offer therapy and skilled nursing for the homebound patient, as well as house call therapy options for the non-homebound. Our team approach to care means you have the support of other therapists, nurses, and staff who are available should you have any concerns or questions. Quarterly team meetings and social events contribute to the culture of community at Great Lakes. Good communication skills, reliability, and timely record keeping are a must. Mileage is compensated. The counties which we serve are Grand Traverse, Southern Antrim, Benzie, Kalkaska, Northern Wexford and Leelanau. Requirements (not an all-inclusive list): Graduate of a PT/PTA accredited university Licensed in the State of Michigan Minimum one year of PT/PTA experience (preferred) Home healthcare experience (preferred) Current, unrestricted professional license CPR/BLS certified Demonstrable knowledge of care and communication skills with the geriatric, disabled, and functional adult populations Ability to use Electronic Medical Record (EMR) software Reliable transportation Current driver's license and auto insurance If you would like to be a member of the Great Lakes team, please apply today!
