Job Description

Renewal Rehab is seeking a compassionate Physical Therapist Assistant to support delivery of high-quality rehabilitative care for seniors in skilled nursing and post-acute settings. Working under the direction of a Physical Therapist, you will implement treatment plans, lead therapeutic exercises, monitor progress, and document outcomes to improve strength, mobility, and independence. Collaborate with an interdisciplinary team in a supportive, patient-centered culture that values integrity, respect, mentorship, and professional growth while positively impacting seniors' daily function and quality of life. Responsibilities Implement PT treatment plans under supervision of a licensed Physical Therapist Provide therapeutic exercises, gait training, balance and functional mobility activities Assist with transfer training and safe use of assistive devices Accurately document treatments, patient progress, and functional outcomes Monitor patient response and report changes to the supervising PT and care team Educate patients, families, and caregivers on exercises and safety strategies Collaborate with an interdisciplinary team to support discharge planning and goals Maintain compliance with federal, state, and facility regulations and policies Contribute to a patient-centered, respectful, and safe therapy environment Participate in continuing education, mentorship, and quality improvement initiatives Required Skills Therapeutic exercise implementation Gait and balance training Functional mobility and transfer training Use of assistive devices and DMEGeriatric and post-acute rehabilitation Clinical documentation and EMR use Patient and family education Infection control and safety practices Range of motion and strengthening techniques Interdisciplinary team collaboration
