Depending on where you live and current patient needs, coverage may also extend into nearby communities such as South Haven and surrounding areas . Rather than expecting one PTA to cover every community in our broader service area, we work to build schedules around geography and patient needs. Use More of Your PTA Skill Set Your patients won't all look the same. You may work with someone rebuilding strength after a hospitalization, another patient recovering from orthopedic surgery, and someone else dealing with balance problems, neurological changes, or progressive functional decline. You'll carry out the supervising PT's plan of care while making observations throughout every visit, monitoring response to treatment, communicating changes, and helping determine whether the patient is moving toward their goals. What You'll Be Doing Provide skilled Physical Therapy treatment in patient homes Implement treatment plans established by the supervising Physical Therapist Provide strengthening, gait, balance, transfer, mobility, and functional training Reinforce individualized home exercise programs Educate patients and caregivers on safe mobility and continued progress Monitor response to treatment and identify meaningful changes Communicate patient progress and concerns to the supervising PT Complete accurate and timely EMR documentation Collaborate with PTs, nurses, OTs, and other interdisciplinary clinicians You Can Be New to Home Health Without Being New to Physical Therapy Experience in Outpatient Therapy, Skilled Nursing, Inpatient Rehabilitation, Transitional Care, Senior Living, Orthopedics, Long-Term Care, or other rehabilitation environments can translate extremely well into home health. We're looking for someone who already understands how to treat patients and is ready to develop the independence, organization, and clinical confidence required in a field-based environment. What You Bring Active Minnesota Physical Therapist Assistant license Valid driver's license, reliable transportation, and automobile insurance Strong hands-on treatment skills Strong patient and caregiver communication Ability to independently manage a field-based schedule Excellent organization and documentation skills Home health experience preferred Experience with geriatric or medically complex patients preferred You Don't Need to Become a PT to Build a Bigger PTA Career If you love being a PTA, there's nothing wrong with wanting to remain one.
