You'll take an established plan of care into the patient's home, provide one-on-one treatment, communicate directly with your supervising PT, and manage your responsibilities out in the community. Independence Without Being on an Island Home health requires confidence. You're walking into the patient's home without a therapy gym full of coworkers around you. You'll need to assess how the patient is responding, adapt within the plan of care, recognize changes, and communicate when something needs the PT's attention. But independent doesn't mean unsupported. You'll:
Provide physical therapy treatments in patient homes Implement established PT plans of care Improve strength, balance, endurance, and functional mobility Provide gait and transfer training Guide therapeutic exercises and home programs Reinforce fall-prevention and safety strategies Educate patients, families, and caregivers Monitor progress and response to treatment Recognize and communicate changes to the supervising PT Complete accurate and timely EMR documentation Travel between patients within your assigned territory Home Health Experience Isn't Required Your experience may come from Outpatient Therapy, Skilled Nursing, Inpatient Rehabilitation, Orthopedics, Transitional Care, Senior Living, Long-Term Care, Hospital-Based Rehabilitation, Geriatrics, or Home Health . What matters is that you've developed the clinical confidence to work independently while knowing when to communicate, collaborate, and ask questions. You'll need a current Minnesota Physical Therapist Assistant (PTA) license , valid driver's license and reliable transportation, strong communication and organization skills, and availability for at least 2 weekdays / 12 visits per week .
