Home Health Care, Inc. is hiring a Physical Therapist Assistant (PTA) to provide one-on-one therapy throughout Buffalo, Delano, Waverly, Maple Lake, Annandale, and surrounding communities . Instead of spending your entire day inside one building, you'll take your PTA skills into patients' homes and help them improve in the environment where mobility actually matters. You'll travel between patient homes, work one-on-one with each person, complete your visit, and move on to the next. Patient schedules are organized around geography, patient needs, and efficient coverage . You won't be expected to bounce between every community in our service area every day. The goal is to create practical field schedules while making sure patients receive the therapy they need. What You'll Do Working under the direction of the supervising Physical Therapist, you'll: Provide skilled Physical Therapy treatment in patient homes Implement individualized plans of care established by the PT Provide strengthening and functional exercise Perform gait and balance training Work on transfers, mobility, and endurance Reinforce individualized home exercise programs Address fall prevention and safe mobility Educate patients and caregivers Monitor patient response and functional progress Communicate meaningful changes to the supervising PT Complete accurate and timely EMR documentation Collaborate with PTs, OTs, nurses, and other interdisciplinary clinicians You Don't Need Home Health on Your Resume to Bring Valuable Experience We're interested in PTAs from Outpatient Therapy, Skilled Nursing, Inpatient Rehabilitation, Orthopedics, Transitional Care, Senior Living, Long-Term Care, Hospital Rehab, Home Health, and other rehabilitation settings . The environment may change, but your skills come with you. If you know how to motivate a patient, safely progress mobility, reinforce a plan of care, recognize when something has changed, and communicate effectively with the supervising PT, you already have a strong foundation for home health. What You Bring Active Minnesota Physical Therapist Assistant (PTA) license Valid driver's license, reliable transportation, and automobile insurance Strong hands-on treatment skills Excellent patient and caregiver communication Ability to independently manage a field-based schedule Strong organization and documentation skills Commitment to patient-centered rehabilitation Home health and experience with geriatric or medically complex patients are preferred, but not required.
