Responsibilities:
Follow treatment plans as directed by the Physical Therapist Provide one-on-one and group therapy to residents Track daily notes, treatment progress, and billing Assist with discharge planning and patient education Communicate with therapy team, nursing, and families Participate in care meetings and facility initiatives
Qualifications:
Graduate of an accredited PTA program Current state licensure as a PTA Prior LTC/SNF experience is a plus, but not required Ability to lift up to 50 lbs and assist residents as needed Strong teamwork and time management skills Join a team committed to providing quality care while creating a positive experience for residents, families, and staff.
