Eventide's Home Health in Kearney and Buffalo County and surrounding areas, NE is seeking a Physical Therapist Assistant to work full time 8a- 4:30p. Tabitha is now Eventide.

Services will be provided to seniors/clients with physical or cognitive disabilities to maximize functional independence. Services are provided in a variety of settings including nursing home, home health care, and other contracted facilities. Essential Functions Reviews senior's/client's chart and obtains current information that may affect Physical Therapist treatment. Seeks guidance from supervising Physical Therapist. Recognizes abnormal physiologic changes and reports them to the supervising Physical Therapist. Performs Physical Therapy treatment procedures as delegated by supervising Physical Therapist in accordance with the initial evaluation and plan of care. Develops and implements the Senior's/client's plan of care in collaboration with the physician, other consulting health care professionals, and the family/caregiver according to agency procedures and state and federal regulations. Actively participates in senior/client care conferences and provides input to the Physical Therapist for developing the plan of care. Actively maintains work schedule which allows for coverage in all Rehab programs as needed. Uses strength, agility, stamina and prioritization skills to respond promptly to frequent and often overlapping senior/client/caregiver/TEAMember/ manager needs or requests. Maintains written documentation reflective of the department standard. Communicates effectively with families/caregivers, TEAMembers, and support services to achieve Senior/client goals and meet service expectations. Ideal Candidate
