Tender Touch Rehab Services is seeking a compassionate Physical Therapist Assistant to provide high-quality rehabilitative care to seniors in skilled nursing and long-term care settings. Working under the direction of a Physical Therapist, you will implement individualized treatment plans, guide therapeutic exercises, support mobility and fall-prevention programs, and document patient progress. You'll collaborate with an interdisciplinary team to promote function, independence, and quality of life. Responsibilities Implement physical therapy treatment plans under PT supervision Provide therapeutic exercises, gait training, and balance activities Assist with transfer training and safe mobility for seniors Monitor, document, and report patient progress and response to treatment Educate patients, families, and caregivers on home exercise and safety Collaborate with interdisciplinary team in skilled nursing/long-term care Adhere to regulatory, documentation, and reimbursement standards Maintain a safe, respectful, and patient-centered care environment Participate in quality improvement and evidence-based practice initiatives Support mentoring and professional development activities as appropriate Required Skills Therapeutic exercise implementation Gait training Balance and fall-prevention techniques Transfer and mobility training Geriatric rehabilitation Skilled nursing facility documentation (EMR) Patient and caregiver education Vital signs monitoring and patient safety Knowledge of PT modalities (e.g., heat, cold, e-stim per PT plan) Understanding of Medicare and post-acute care regulations
