Job Description
Tender Touch Rehab Services is seeking a dedicated Physical Therapist Assistant to join our Healthcare & Medical Services team. In this role, you will provide high-quality therapy services to seniors in skilled nursing and long-term care settings, implementing treatment plans under the direction of a Physical Therapist. You will help improve residents' function, independence, and quality of life while collaborating with an interdisciplinary team. Responsibilities Deliver one-on-one physical therapy treatments under supervision of licensed PTFollow and progress established plans of care to improve mobility, strength, and balance Assist with gait training, transfers, and functional mobility for seniors Accurately document treatments, patient responses, and progress in EMREducate patients, families, and caregivers on home exercise and safety Participate in interdisciplinary team meetings and care planning Ensure patient safety, dignity, and comfort during all interventions Support quality, compliance, and evidence-based practice standards Required Skills Therapeutic exercise implementation Gait training and balance interventions Modalities (heat, cold, e-stim as allowed by state law) Treatment documentation and EMR use Patient and family education Functional mobility training (transfers, bed mobility) Vital signs monitoring and safety awareness Interdisciplinary team collaboration Care planning in skilled nursing/LTCTime management and caseload prioritization
