Job Description

Tender Touch Rehab Services is seeking a compassionate Physical Therapist Assistant to provide hands-on care for seniors in skilled nursing and long-term care facilities. In this role, you'll implement individualized treatment plans designed by Physical Therapists, focusing on mobility, strength, balance, and functional independence for geriatric and sub-acute residents. You'll assist with therapeutic exercises, gait training, transfers, and use of assistive devices while carefully documenting progress in EMR systems. Working in our collaborative, patient-focused culture, you'll partner closely with an interdisciplinary team and facility staff to deliver high-quality, dignified care. Responsibilities Implement physical therapy treatment plans under the supervision of a Physical Therapist Provide therapeutic exercises, gait training, and functional mobility interventions for geriatric and sub-acute residents Assist residents with safe transfers, positioning, and use of assistive devices and durable medical equipment Accurately document treatment sessions, resident responses, and progress in EMR systems Monitor resident safety and adhere to fall-prevention and infection-control protocols Communicate effectively with PTs, nursing staff, and other interdisciplinary team members regarding resident status Educate residents and families on exercises, safety techniques, and use of equipment to support carryover Participate in care plan meetings and contribute to goal setting and treatment progression Support a compassionate, resident-centered environment aligned with facility and company standards Engage in ongoing professional development and training offered by Tender Touch Rehab Services Required Skills Therapeutic exercise implementation Gait and balance training Transfer and mobility assistance Use of assistive devices and DMETherapeutic modalities application Documentation in EMR systems Care plan implementation Patient and family education Safety and fall-prevention techniques Interdisciplinary team collaboration
