Job Description
Tender Touch Rehab Services is seeking a dedicated Physical Therapist Assistant to support high-quality rehab services for geriatric residents in skilled nursing and long-term care settings. In this role, you'll implement treatment plans designed by Physical Therapists, providing hands-on interventions to improve mobility, strength, balance, and functional independence. You'll document progress, assist with care planning, and collaborate closely with an interdisciplinary team in a compassionate, patient-focused environment that emphasizes clinical excellence, professional growth, and open communication. Responsibilities Implement Physical Therapist treatment plans for geriatric residents in skilled nursing and long-term care settings Provide therapeutic exercises, gait training, balance activities, and functional mobility interventions Assist residents with safe transfers and use of assistive devices, following proper body mechanics Accurately document treatments, resident progress, and responses in EMR systems Communicate patient status and recommendations to Physical Therapists and interdisciplinary team members Educate residents and families on exercises, safety techniques, and use of equipment Support discharge planning by providing input on functional status and equipment needs Adhere to clinical, safety, and infection control standards and regulatory requirements Participate in team meetings and contribute to individualized care plans Engage in ongoing professional development and training offered by the organization Required Skills Therapeutic exercise implementation Gait and balance training Modalities application (heat, e-stim, ultrasound) Patient transfer and safe mobility techniques Treatment documentation in EMR systems Care plan implementation and progression Knowledge of geriatric rehabilitation Interdisciplinary team collaboration Patient and family education Infection control and safety procedures
