Job Description

Tender Touch Rehab Services is seeking a compassionate Certified Physical Therapist Assistant to join our Healthcare & Medical Services team, providing individualized therapy to seniors in skilled nursing and long-term care settings. In this on-site role, you will implement PT treatment plans focused on improving mobility, function, safety, and quality of life. Responsibilities include therapeutic exercise, gait and balance training, functional activities, accurate EMR documentation, and education for patients and caregivers. You will collaborate closely with an interdisciplinary team in a supportive, patient-centered culture that values dignity, respect, and clinical excellence. Responsibilities Deliver compassionate, evidence-based physical therapy interventions to seniors in skilled nursing and long-term care settings Implement and progress PT plans of care under the supervision of a Physical Therapist to improve function, mobility, and safety Provide gait, balance, strengthening, and functional training focused on maximizing independence and quality of life Document treatments, patient responses, and progress accurately and timely in the EMR system Collaborate closely with PTs, nursing, and interdisciplinary team members to support coordinated, patient-centered care Educate patients, families, and caregivers on exercises, safety strategies, and equipment use Contribute to discharge planning and restorative or maintenance programs to support long-term outcomes Participate in clinical education, mentorship, and quality initiatives to support professional growth and clinical excellence Required Skills Therapeutic exercise and gait training Knowledge of geriatric conditions and fall prevention Safe patient handling and transfer techniques Therapeutic modalities (e.g., heat, cold, e-stim) Care plan implementation and progression Functional mobility and balance training Clinical documentation in EMR systems Patient and family education Interdisciplinary team collaboration Outcome measurement and restorative programs
