Job Description

Tender Touch Rehab Services is seeking a compassionate Physical Therapist Assistant to provide high-quality, evidence-based therapy for seniors in skilled nursing and long-term care settings. Working under the supervision of a Physical Therapist, you will implement individualized treatment plans to improve mobility, function, and independence, while maintaining accurate documentation and collaborating with an interdisciplinary team. Responsibilities Implement individualized PT treatment plans for senior patients under PT supervision Provide therapeutic exercises, gait training, balance and mobility interventions to improve function and independence Assist with transfer training and safe use of assistive devices and equipment Monitor patient progress, communicate changes to supervising PT, and contribute to updates of care plans Document all treatments, patient responses, and progress accurately and timely in the EMR system Collaborate with nursing, rehab, and medical teams to coordinate comprehensive, patient-centered care Educate patients, families, and caregivers on home exercises, safety, and strategies to maintain function Support a culture of dignity, compassion, and respect for all residents and patients Participate in quality improvement, in-services, and continuing education to enhance clinical skills Adhere to all regulatory, ethical, and company standards for care delivery in post-acute settings. Required Skills Therapeutic exercise implementation Gait training and balance interventions Transfer and mobility training Use of assistive devices and DMETherapeutic modalities (e.g., heat, cold, e-stim) as allowed by state practice act Functional mobility and ADL-focused interventions Accurate therapy documentation/EMR use Care planning in skilled nursing/long-term care Patient and family education Interdisciplinary team collaboration
