We're seeking a licensed Physical Therapist Assistant (PTA) to join our growing rehabilitation team in a Skilled Nursing Facility (SNF) in Avon, Ohio. This role offers the opportunity to work with older adults in a collaborative post-acute care environment focused on improving mobility, independence, safety, and quality of life. Corporate Discount Programs Requirements What You'll Do Provide physical therapy treatments in accordance with individualized care plans Help patients improve mobility, strength, balance, endurance, and functional independence Communicate patient progress and recommendations to the supervising Physical Therapist and interdisciplinary care team Collaborate with nursing staff, physicians, families, and caregivers Participate in care planning and discharge planning activities Educate patients, families, and facility staff on therapy programs and safety strategies Maintain timely and accurate electronic documentation and billing records Assist with restorative and functional maintenance programs Recommend equipment and treatment interventions that support successful patient outcomes Support a positive rehabilitation environment focused on quality care and service excellence Qualifications Required Graduate of an accredited Physical Therapist Assistant program Current PTA license in the State of Ohio Strong communication and interpersonal skills Commitment to patient-centered care Ability to work collaboratively within an interdisciplinary healthcare team Preferred Experience in Skilled Nursing, Rehabilitation, Long-Term Care, Post-Acute Care, or Geriatrics New graduates are encouraged to apply Compensation & Career Growth Concept Rehab is committed to offering a highly competitive compensation and benefits package designed to attract and retain top rehabilitation professionals.
